1. Ritonavir

HIV protease inhibitor now used almost entirely as a booster: in small doses it blocks CYP3A4, raising levels of other drugs such as darunavir and lopinavir. It is the "r" in combinations written as "DRV/r" and is part of Paxlovid for COVID-19.
Example: Paxlovid: nirmatrelvir 300 mg with RTV 100 mg BID x 5 days; hold simvastatin.
Because of strong CYP3A4 inhibition, ritonavir causes many serious drug interactions, for example with statins, some antiarrhythmics and transplant drugs, so every Paxlovid prescription requires a medication review. Its pharmacokinetic-boosting role was an unexpected repurposing of a drug originally approved in 1996 at higher, poorly tolerated doses.
