1. National coverage determination

Medical · Health Insurance Abbreviations

Medicare decision, made by CMS through a public process, on whether a specific item or service is covered nationwide and under what conditions. NCDs bind all Medicare Administrative Contractors and Medicare Advantage plans.

Example: The NCD covers TAVR only when performed by a qualified heart team.

Some NCDs use "coverage with evidence development", which covers a service only for patients enrolled in approved studies or registries. A national determination supersedes conflicting local ones. Requests for new NCDs can come from manufacturers, clinicians or patient groups.

2. Neurocognitive disorder

Medical · Mental Health & Psychiatry Abbreviations

DSM-5 category for acquired decline in cognitive functions such as memory, attention, language or executive function. Major NCD corresponds to dementia, mild NCD to mild cognitive impairment, and delirium is also grouped here; subtypes name the cause, such as Alzheimer's disease or vascular disease.

Example: Major NCD due to Alzheimer's disease, moderate; caregiver support and safety assessment.

DSM-5 adopted "neurocognitive disorder" in 2013 partly to reduce stigma attached to "dementia" and to include younger people with brain injury or HIV. In public health and WHO documents NCD far more commonly means noncommunicable disease, such as heart disease, cancer and diabetes, so the source of the document usually settles the meaning.

3. No-claims discount

Business & Finance · Insurance Abbreviations

Premium discount, used in British and many other motor insurance markets, that grows for each consecutive year a driver goes without making a claim, often reaching 60% or more after five years.

Example: After nine claim-free years, my NCD knocks most of the price off my car insurance.

Also called no-claims bonus (NCB). Drivers can usually pay extra for "protected NCD," which preserves the discount after one or two claims, though the base premium may still rise.
