TRD stands for Treatment-resistant depression, a mental health & psychiatry abbreviation.
Major depression that has not responded adequately to at least two antidepressant trials of adequate dose and duration. Options include augmentation with other drugs, switching classes, esketamine nasal spray, TMS and ECT.
Example: TRD after sertraline, venlafaxine and bupropion augmentation; start esketamine twice weekly in certified clinic.
Usage notes
Esketamine nasal spray (Spravato) was approved by the FDA for TRD in 2019 and must be given under observation because of dissociation and sedation. The large STAR*D trial showed that remission becomes less likely with each successive treatment step. Ketamine infusions are used off-label. Pronounced as letters.
