TRALI stands for Transfusion-related acute lung injury, a hematology and oncology abbreviation.
Sudden noncardiogenic pulmonary edema causing severe low oxygen within six hours of a blood transfusion. It is usually caused by donor antibodies against the recipient's white blood cells and has been one of the leading causes of transfusion-related deaths.
Example: Acute hypoxemia and bilateral infiltrates 2 hours after FFP, normal BNP: suspected TRALI, reported to blood bank.
Usage notes
Plasma-rich products from women who have been pregnant are more likely to contain the responsible HLA or neutrophil antibodies, so blood services began favoring male donors for plasma in the mid-2000s, sharply reducing cases. Treatment is supportive, and most patients improve within days. It must be distinguished from TACO, which is caused by fluid overload.
