TACO stands for Transfusion-associated circulatory overload, a hematology and oncology abbreviation.
Pulmonary edema caused by transfusing blood products faster or in greater volume than the circulation can handle, typically in older adults or patients with heart or kidney disease. Signs include breathlessness, high blood pressure, raised jugular venous pressure and elevated BNP.
Example: SOB and BP 178/94 after second unit; crackles, BNP 1,600: TACO, furosemide given and transfusion slowed.
Usage notes
TACO is now among the most commonly reported causes of transfusion-related deaths. Prevention includes transfusing one unit at a time with reassessment, slower infusion rates and diuretics in high-risk patients. Unlike TRALI, it responds to diuretics and is associated with hypertension rather than hypotension.
