VRd stands for Velcade (bortezomib), Revlimid (lenalidomide), dexamethasone, a hematology and oncology abbreviation.
Three-drug induction regimen for newly diagnosed multiple myeloma, combining a proteasome inhibitor, an immunomodulatory drug and a steroid. It is used before autologous stem cell transplant or as continuing therapy for patients not going to transplant.
Example: Newly diagnosed myeloma: VRd induction x4 cycles, then stem cell collection.
Usage notes
The SWOG S0777 trial (2017) showed that adding bortezomib to lenalidomide and dexamethasone improved both progression-free and overall survival. Adding the anti-CD38 antibody daratumumab (D-VRd) further deepened responses in later trials. Bortezomib can cause nerve damage, reduced by giving it under the skin weekly, and patients receive antiviral prophylaxis against shingles. Also written RVD.
