PCSK9i stands for PCSK9 inhibitor, a drug class abbreviation.

Cholesterol-lowering drugs that block proprotein convertase subtilisin/kexin type 9, increasing LDL receptors on the liver. Evolocumab and alirocumab are monoclonal antibodies injected every two or four weeks; inclisiran is a small interfering RNA given twice yearly.

Example: LDL 112 on max statin and ezetimibe after MI; add PCSK9i evolocumab 140 mg q2wk.

Usage notes

Evolocumab and alirocumab, both approved in 2015, lower LDL cholesterol by about 60 percent, and the FOURIER and ODYSSEY OUTCOMES trials showed fewer cardiovascular events. The target was discovered through families with genetic mutations causing very high or very low cholesterol. High prices initially required prior authorization for most patients.
