PARPi stands for Poly(ADP-ribose) polymerase inhibitor, a drug class abbreviation.
Oral targeted cancer drugs, olaparib, niraparib, rucaparib and talazoparib, that block a DNA repair enzyme. Cancer cells with BRCA1, BRCA2 or similar repair defects cannot survive the extra DNA damage, a concept called synthetic lethality.
Example: BRCA2-mutated metastatic prostate cancer: start PARPi olaparib 300 mg BID.
Usage notes
Olaparib was approved in 2014 as the first PARP inhibitor, for BRCA-mutated ovarian cancer, and the class now treats breast, prostate and pancreatic cancers. Anemia and fatigue are common, and a small risk of myelodysplastic syndrome and acute myeloid leukemia is noted on labels. Some ovarian indications were later narrowed based on survival data.
