P2Y12i stands for P2Y12 inhibitor, a drug class abbreviation.
Antiplatelet drugs that block the P2Y12 receptor on platelets: clopidogrel, prasugrel, ticagrelor and IV cangrelor. Combined with aspirin as dual antiplatelet therapy, they prevent stent thrombosis and heart attacks after coronary stenting.
Example: Load P2Y12i ticagrelor 180 mg, then 90 mg BID with aspirin 81 mg.
Usage notes
Clopidogrel is a prodrug activated by CYP2C19, so poor metabolizers get less benefit. Prasugrel is contraindicated in people with a prior stroke or TIA, and ticagrelor's benefit is reduced if aspirin exceeds 100 mg daily, as stated in its boxed warning. Stopping these drugs early after a stent raises the risk of stent thrombosis.
