HSIL stands for High-grade squamous intraepithelial lesion, an obstetrics & gynecology abbreviation.
Pap result indicating moderate to severe cell changes with a substantial risk of underlying CIN 2 or CIN 3, the true cancer precursors. It requires colposcopy or, in some cases, immediate excisional treatment.
Example: Pap HSIL with HPV 16 positive; expedited LEEP offered per ASCCP risk estimate.
Usage notes
ASCCP guidelines allow treatment without a confirming biopsy when the estimated risk of CIN 3 or worse is high. HSIL in pregnancy is evaluated with colposcopy, but treatment is usually delayed until after delivery. Pronounced as letters or "AITCH-sil".
