Treatment of male hypogonadism with testosterone gels, injections, patches, pellets or oral capsules. Guidelines require symptoms plus consistently low morning testosterone, generally below about 300 ng/dL on two tests, before starting.
Example: Total T 212 and 238 ng/dL with fatigue and low libido; start TRT gel, check hematocrit in 3 months.
Monitoring includes hematocrit, because testosterone can cause erythrocytosis, and PSA in older men. TRT suppresses sperm production and is avoided in men seeking fertility. The large TRAVERSE trial (2023) found no increase in major cardiovascular events in men with hypogonadism. Gel transfer to children and partners carries a boxed warning.
2. Turkey Time
