CategoriesMedical

SIMV stands for Synchronized intermittent mandatory ventilation, a respiratory abbreviation.

Ventilator mode that delivers a set number of mandatory breaths per minute, timed to coincide with the patient's own efforts, while allowing spontaneous breaths in between that may receive pressure support. It was once widely used for weaning.

Example: SIMV rate 8, Vt 450, PS 10, PEEP 5; spontaneous rate 14.

Usage notes

Trials in the 1990s by Brochard and Esteban found that weaning with SIMV took longer than using pressure support or daily spontaneous breathing trials, so it has fallen out of favor in adults. It remains in use in some pediatric and anesthesia settings. Spontaneous breaths without support can increase the work of breathing.
