CategoriesMedical

PTLD stands for Post-transplant lymphoproliferative disorder, a hematology and oncology abbreviation.

Spectrum of lymphoid growths, from benign proliferations to aggressive lymphoma, that develop after solid organ or stem cell transplantation because of immunosuppression. Most are driven by Epstein-Barr virus.

Example: Kidney transplant recipient with rising EBV load and new abdominal mass: PTLD, tacrolimus reduced, rituximab started.

Usage notes

Risk is highest in the first year after transplant and in EBV-negative recipients of EBV-positive organs, which is common in children. First steps include reducing immunosuppression and giving rituximab, with chemotherapy for aggressive forms. EBV-specific T-cell therapy (tabelecleucel) was approved in Europe in 2022 for relapsed EBV-positive PTLD.
