1. Progressive supranuclear palsy

Rare neurodegenerative disease caused by tau protein buildup, producing early falls, stiffness of the neck and trunk, slowed speech and swallowing, and a characteristic difficulty moving the eyes up and down. It is often initially mistaken for Parkinson's disease.
Example: Unexplained backward falls, vertical gaze palsy and poor levodopa response: probable PSP.
First described as a distinct disease by Steele, Richardson and Olszewski in 1964. MRI can show midbrain atrophy, the "hummingbird sign." There is no disease-modifying treatment, and levodopa usually helps little. Comedian Dudley Moore had PSP. The 2017 Movement Disorder Society criteria recognize several clinical variants.
