NICM stands for Nonischemic cardiomyopathy, a cardiology abbreviation.
Ventricular dysfunction not explained by coronary artery disease, a catch-all covering dilated, genetic, viral, toxic, infiltrative, peripartum and tachycardia-induced causes. Patterns of scarring on cardiac MRI help separate it from ischemic disease and point to specific diagnoses.
Example: Cath with clean coronaries: NICM, EF 25%; check TSH, iron studies and HIV.
Usage notes
The DANISH trial (2016) found that prophylactic ICDs in NICM did not significantly reduce overall mortality, although younger patients appeared to benefit, so device decisions are more individualized than in ischemic disease. Myocarditis, sarcoidosis, iron overload and alcohol-related disease are treatable causes worth identifying early.
