MSIR stands for Morphine sulfate immediate release, a drug abbreviation.
Short-acting oral morphine tablets or solution used for acute pain and for breakthrough pain in patients on long-acting opioids. Effects begin within about 30 minutes and last about 4 hours.
Example: MSIR 15 mg PO q4h PRN breakthrough pain; continue MS Contin 30 mg q12h.
Usage notes
The abbreviation persists because it distinguishes immediate-release morphine from extended-release forms such as MS Contin. Because "MS" is on The Joint Commission "Do Not Use" list, many hospitals require "morphine IR" or "morphine immediate-release" in orders. Oral morphine solution comes in 2 mg/mL and 20 mg/mL strengths, a known source of overdoses.
