HFpEF stands for Heart failure with preserved ejection fraction, a cardiology abbreviation.
Heart failure symptoms with an ejection fraction of 50 percent or more, caused by a stiff ventricle that fills poorly rather than weak pumping. It accounts for about half of heart failure cases and is linked to older age, female sex, obesity, hypertension, diabetes and atrial fibrillation.
Example: Echo: LVEF 60%, grade II diastolic dysfunction, elevated filling pressures; findings fit HFpEF in the setting of obesity and HTN.
Usage notes
Pronounced "HEF-pef" and formerly called diastolic heart failure. Diagnosis can be difficult because the ejection fraction looks normal; scores such as H2FPEF and HFA-PEFF, natriuretic peptides and exercise testing help. Few drugs improved outcomes until SGLT2 inhibitors showed benefit in trials published in 2021 and 2022, and GLP-1-based drugs have since helped obese patients.
