HOCM stands for Hypertrophic obstructive cardiomyopathy, a cardiology abbreviation.
Form of hypertrophic cardiomyopathy in which the thickened septum and forward motion of the mitral valve block blood leaving the left ventricle. Obstruction means an outflow gradient of 30 mmHg or more at rest or with provocation; 50 mmHg or more is the usual threshold for invasive treatment.
Example: HOCM with gradient rising to 90 mmHg on Valsalva; referred for septal myectomy.
Usage notes
Pronounced "HOE-kum." The murmur grows louder with Valsalva and standing, which reduce preload, and softer with squatting or handgrip, a classic bedside distinction from aortic stenosis. Dehydration, vasodilators and nitrates can worsen obstruction. An older name was IHSS, idiopathic hypertrophic subaortic stenosis.
