GnRHa stands for Gonadotropin-releasing hormone agonist, a drug class abbreviation.

Drugs such as leuprolide (Lupron), goserelin, triptorelin and histrelin that, after an initial surge, shut down pituitary signals to the testes and ovaries. They treat prostate cancer, endometriosis, uterine fibroids and central precocious puberty, and are used in IVF and gender-affirming care.

Example: Leuprolide (GnRHa) 22.5 mg IM q3 months plus bicalutamide x 2 weeks for flare.

Usage notes

The initial testosterone surge ("flare") can worsen bone pain or urinary blockage in advanced prostate cancer, so an antiandrogen is often given for the first weeks. GnRH antagonists such as degarelix and oral relugolix (approved in 2020) lower hormones immediately without a flare. Long-term use causes hot flashes and bone loss.
