CVVHDF stands for Continuous venovenous hemodiafiltration, a nephrology & urology abbreviation.
CRRT mode that combines convection with replacement fluid and diffusion with dialysate, clearing both small and middle-sized molecules. It is a common default ICU modality for unstable patients with acute kidney injury.
Example: Order: CVVHDF, dialysate 1,500 mL/h, replacement 1,000 mL/h, blood flow 200 mL/min.
Usage notes
Combining both mechanisms gives flexible control over total effluent dose. Drug dosing for antibiotics such as vancomycin and piperacillin must be adjusted because CRRT removes them. Circuit clotting and electrolyte disturbances such as low phosphate are frequent issues.
