CRF stands for Case report form, a medical research abbreviation.

Printed or electronic document designed to record all protocol-required information on each trial participant, such as eligibility, visit data, lab results, adverse events and study drug dosing. The electronic version is called an eCRF.

Example: Adverse event logged on the CRF with onset date, grade and relationship to study drug.

Usage notes

CRF design affects data quality, so standards such as CDISC CDASH define common data fields. Every entry must be traceable to a source document. In nephrology CRF can mean chronic renal failure, an older name for chronic kidney disease.
