WTD stands for Wet-to-dry dressing, a nursing abbreviation.
Traditional dressing in which saline-moistened gauze is packed into a wound, allowed to dry and then pulled off, removing dead tissue stuck to it. It works as non-selective mechanical debridement.
Example: Packed wound with NS WTD, 4 x 4 gauze, BID changes; premedicated for pain.
Usage notes
Also written W-D or "wet to dry". Because it is painful, cools the wound and strips healthy tissue along with dead tissue, wound care specialists largely prefer moisture-retentive dressings, enzymatic agents or NPWT. Gauze should be only moist, not dripping, to avoid soaking the surrounding skin.
