Motor nerve cell whose cell body lies in the brain and whose axon runs down to the spinal cord, controlling lower motor neurons. Damage causes weakness with spasticity, brisk reflexes, an upgoing big toe (Babinski sign) and little muscle wasting.
Example: Hyperreflexia, spasticity and bilateral Babinski signs: UMN pattern, MRI cervical spine ordered.
Stroke, multiple sclerosis, spinal cord compression and ALS produce upper motor neuron signs. Recognizing whether weakness follows an upper or lower motor neuron pattern is one of the first steps in localizing a neurological problem. ALS is unusual in showing both patterns at once.
