qSOFA stands for Quick Sequential Organ Failure Assessment, an emergency medicine abbreviation.

Bedside sepsis risk tool with three criteria: respiratory rate of 22 or more, altered mentation and systolic blood pressure of 100 or less. Two or more points suggest higher risk of poor outcome in suspected infection.

Example: qSOFA 2: RR 24, new confusion; lactate and cultures sent.

Usage notes

Introduced with the Sepsis-3 definitions in 2016. The 2021 Surviving Sepsis Campaign guidelines recommend against using qSOFA alone as a screening tool because it misses many septic patients. The full SOFA score measures six organ systems in intensive care.
