P-gp stands for P-glycoprotein, a pharmacology abbreviation.
Transporter protein that pumps drugs out of cells in the gut wall, kidneys, liver and blood-brain barrier, limiting absorption and brain entry. Digoxin, dabigatran and some HIV drugs are P-gp substrates.
Example: Amiodarone inhibits P-gp, so reduce the digoxin dose by about half.
Usage notes
Encoded by the ABCB1 gene (also called MDR1), P-gp was first discovered as a cause of multidrug resistance in cancer cells. Many CYP3A4 inhibitors also block P-gp, which compounds interactions. Some dog breeds, such as collies, carry MDR1 mutations that make ivermectin toxic to them.
