CategoriesMedical

LMWH stands for Low-molecular-weight heparin, a drug class abbreviation.

Injectable anticoagulants, mainly enoxaparin (Lovenox) and dalteparin (Fragmin), made by breaking heparin into smaller fragments. Given subcutaneously once or twice daily, they prevent and treat deep vein thrombosis and pulmonary embolism.

Example: LMWH (enoxaparin 40 mg subcut daily) for VTE prophylaxis post hip replacement.

Usage notes

Doses are weight-based and must be reduced in severe kidney impairment (creatinine clearance below 30 mL/min). LMWHs have a lower risk of heparin-induced thrombocytopenia than unfractionated heparin, and they are the preferred anticoagulant in pregnancy because they do not cross the placenta. Protamine only partially reverses them.
