BPMH stands for Best possible medication history, a pharmacy abbreviation.
Structured medication history gathered from at least two sources, such as patient interview, pill bottles, pharmacy fill records and outpatient notes, used as the foundation for medication reconciliation.
Example: BPMH sources: patient, wife, CVS fill history and PCP list; discrepancy on insulin dose.
Usage notes
The term was popularized by ISMP Canada and the WHO High 5s project on medication reconciliation. A BPMH differs from a quick "med list" because it verifies what the patient is actually taking, not just what was prescribed. Studies find unintended discrepancies in a large share of hospital admissions without it.
