ASPECTS stands for Alberta Stroke Program Early CT Score, a neurology abbreviation.
Ten-point score for early ischemic changes on noncontrast CT in the middle cerebral artery territory: one point is subtracted for each of ten regions showing damage. A score of 10 is normal, and lower scores indicate larger infarcts.
Example: CT ASPECTS 8 with left M1 occlusion; proceed to thrombectomy.
Usage notes
Introduced by Barber and colleagues in 2000. Thrombectomy trials generally required an ASPECTS of 6 or more, but trials published in 2023, such as SELECT2 and ANGEL-ASPECT, found benefit even in patients with large infarcts scoring 3 to 5. The score can also be applied to CT perfusion and MRI diffusion images.
