TORB stands for Telephone order read back, a medical documentation abbreviation.
Charting tag indicating that a telephone order was repeated back to the prescriber and confirmed. It is common in long-term care and home health, where prescribers are often off site.
Example: Hold metformin for CT with contrast, TORB Dr. Ortiz/M. Reyes LPN.
Usage notes
Includes the prescriber's name, the receiving nurse and the time. Some forms use "T.O./R.B." Read-back catches errors such as wrong drug, dose or patient before they reach the patient. The prescriber later signs to authenticate the order.
