UTD stands for Up to date, a medical documentation abbreviation.

Notation that the patient's immunizations or screenings are current, recorded in pediatric visits, triage and preventive care notes. It is most often applied to vaccines, tetanus status and age-appropriate cancer screening.

Example: Immunizations UTD per state registry; flu vaccine given today.

Usage notes

"Tetanus UTD" in an emergency note means a booster within the recommended interval, generally 5 years for dirty wounds and 10 years otherwise. Pediatric notes often confirm UTD against state immunization registries. In medical education, UpToDate is also the name of a point-of-care reference.
