Focus-charting format in nursing in which each entry names a focus, such as a symptom, event or behavior, then records supporting data, the nursing action taken and the patient's response.
Example: DAR note, focus anxiety. D: pacing, HR 104. A: quiet room, breathing coaching. R: calmer, HR 86.
Part of Focus Charting, introduced in the early 1980s to move nursing notes away from long chronological narratives. The focus need not be a nursing diagnosis, which makes the method flexible. Some institutions use DARP, adding P for plan.
2. Daughters of the American Revolution

Lineage society for women who can prove descent from someone who aided American independence, devoted to historic preservation, education and patriotism. Its headquarters in Washington includes Constitution Hall and a large genealogy library.
Example: She joined the DAR after tracing her line to a Revolutionary War private.
Founded in 1890, it has more than 180,000 members. In 1939 it barred the Black contralto Marian Anderson from singing at Constitution Hall, prompting Eleanor Roosevelt to resign and leading to Anderson's historic concert at the Lincoln Memorial. The organization has since acknowledged and apologized for that history.
