T&P stands for Turn and position, a nursing abbreviation.

Nursing intervention of repositioning a patient who cannot move independently, traditionally every two hours, to relieve pressure over bony areas and prevent pressure injuries, pneumonia and contractures. Each turn is charted with the new position.

Example: T&P q2h: 0200 left side with pillows, heels floated, skin intact.

Usage notes

The two-hour interval is tradition rather than a fixed evidence-based number; pressure-redistributing mattresses may allow longer intervals for some patients. A 30-degree lateral tilt offloads the sacrum and hip bones better than a full side-lying position. Turning is often combined with coughing and deep breathing as TCDB.
