RVH stands for Right ventricular hypertrophy, a cardiology abbreviation.
Thickening of the right ventricular wall from pressure overload, as in pulmonary hypertension, pulmonary stenosis, chronic lung disease or congenital defects such as tetralogy of Fallot. ECG clues include right axis deviation, a tall R wave in V1 and right atrial enlargement.
Example: Right axis deviation and dominant R in V1 suggesting RVH; echo shows RVSP 68 mmHg.
Usage notes
Cor pulmonale is the term for right ventricular enlargement or failure caused by lung disease. ECG detection of RVH is insensitive, especially in adults with COPD, whose hyperinflated lungs dampen voltages. In newborns a dominant right ventricle is normal because the right side does more work before birth.
