MIVF stands for Maintenance intravenous fluids, a nursing abbreviation.
IV fluids given at a rate meant to cover a patient's ongoing daily water and electrolyte needs when they cannot eat or drink enough, as opposed to boluses that replace a deficit. Rates are calculated from weight, especially in children.
Example: MIVF D5 1/2NS + 20 KCl at 80 mL/hr while NPO.
Usage notes
Pediatric rates are classically estimated with the Holliday-Segar "4-2-1" rule: 4 mL/kg/hr for the first 10 kg, 2 for the next 10 and 1 for each kilogram above 20. A 2018 American Academy of Pediatrics guideline recommends isotonic fluids for most children to reduce the risk of hospital-acquired hyponatremia.
