MGUS stands for Monoclonal gammopathy of undetermined significance, a hematology and oncology abbreviation.
Common premalignant condition in which plasma cells produce a small amount of an abnormal antibody (M-protein), below 3 g/dL, with fewer than 10 percent plasma cells in the marrow and no organ damage. It affects about 3 percent of adults over 50.
Example: Incidental M-spike 0.8 g/dL IgG kappa on SPEP: low-risk MGUS, repeat labs in 6 months.
Usage notes
MGUS progresses to multiple myeloma or a related disorder at about 1 percent per year, so patients are monitored with periodic blood and urine tests rather than treated. The concept was defined by Robert Kyle at the Mayo Clinic in 1978. Risk models use M-protein size, type and the free light chain ratio. Some MGUS proteins cause kidney or nerve damage despite low levels, termed monoclonal gammopathy of clinical significance.
