IVH stands for Intraventricular hemorrhage, a neurology abbreviation.

Bleeding into the fluid-filled ventricles of the brain. In premature babies it arises from the fragile germinal matrix and is graded I to IV, while in adults it usually results from extension of an intracerebral or subarachnoid hemorrhage and can block CSF flow, causing hydrocephalus.

Example: Head ultrasound on day 3 shows grade III IVH in a 26-week preterm infant.

Usage notes

The Papile grading system for premature infants (1978) ranges from germinal matrix bleeding only (grade I) to bleeding extending into brain tissue (grade IV). Risk is highest in the first 72 hours of life and in the most premature babies. In adults, an external ventricular drain manages hydrocephalus, and the CLEAR III trial (2016) studied clot-dissolving drugs given through the drain.
