FENa stands for Fractional excretion of sodium, a nephrology & urology abbreviation.
Calculation comparing how much filtered sodium appears in the urine, used to separate prerenal acute kidney injury from tubular damage. Values below 1 percent suggest the kidneys are avidly retaining sodium (prerenal), while values above 2 percent suggest acute tubular necrosis.
Example: FENa 0.4 percent with BUN/Cr 28: prerenal AKI, start isotonic fluids.
Usage notes
Calculated as (urine sodium × plasma creatinine) ÷ (plasma sodium × urine creatinine) × 100. Diuretics make FENa unreliable, so the fractional excretion of urea (below about 35 percent suggests prerenal) is used instead. Pronounced "fee-NA" or "F-E-N-A".
