EVT stands for Endovascular thrombectomy, a neurology abbreviation.
Catheter procedure that removes a clot from a large brain artery, usually with a stent retriever or aspiration catheter, threaded from the groin or wrist. It dramatically improves outcomes for strokes caused by large vessel occlusion when performed within 6 hours, and up to 24 hours in selected patients.
Example: Left M1 occlusion, ASPECTS 9: EVT achieved eTICI 2c reperfusion in one pass.
Usage notes
Five randomized trials published in 2015, including MR CLEAN, ESCAPE and SWIFT PRIME, proved its benefit; a pooled analysis (HERMES) found that roughly one in three treated patients had less disability. DAWN and DEFUSE 3 (2018) extended the window to 24 hours using imaging selection. It is often given after intravenous thrombolysis. Performed by neurointerventionalists at thrombectomy-capable and comprehensive stroke centers.
