BPaL stands for Bedaquiline, pretomanid and linezolid, a drug abbreviation.
Six-month all-oral regimen for highly drug-resistant tuberculosis, replacing older treatments that lasted up to two years and required daily injections. Adding moxifloxacin creates BPaLM.
Example: XDR-TB: start BPaL x 26 weeks; monitor CBC and vision on linezolid.
Usage notes
The regimen was based on the Nix-TB trial, which led to FDA approval of pretomanid in 2019, and the WHO recommended BPaLM for most multidrug-resistant TB in 2022. Linezolid toxicity, including nerve damage and low blood counts, is the main problem, and later trials found lower doses effective.
