TIVA stands for Total intravenous anesthesia, an anesthesia abbreviation.
General anesthesia induced and maintained entirely with intravenous drugs, typically a propofol infusion combined with an opioid such as remifentanil, without inhaled anesthetic gases. It is chosen for patients at high risk of PONV or susceptible to malignant hyperthermia, for neuromonitoring cases and for some airway surgery.
Example: TIVA with propofol and remifentanil to allow motor evoked potential monitoring.
Usage notes
Because there is no exhaled gas concentration to follow, processed EEG monitoring is commonly used to reduce the risk of awareness. Outside the US, target-controlled infusion pumps automate dosing. TIVA also avoids the greenhouse gas emissions of inhaled anesthetics, a growing consideration in sustainable anesthesia practice.
