TcB stands for Transcutaneous bilirubin, a pediatric & neonatal abbreviation.
Noninvasive estimate of a newborn's bilirubin level obtained by shining light on the skin with a handheld meter, used to screen for jaundice before discharge. Results near treatment thresholds are confirmed with a blood test (total serum bilirubin).
Example: TcB 11.2 at 40 hours, below phototherapy threshold; recheck at follow-up in 48 hours.
Usage notes
The AAP's 2022 hyperbilirubinemia guideline raised phototherapy thresholds slightly and uses hour-specific nomograms based on gestational age and risk factors. TcB is less accurate after phototherapy has started and may be affected by skin pigmentation. Pronounced as letters.
