PI-RADS stands for Prostate Imaging Reporting and Data System, a medical imaging abbreviation.
Scoring system from 1 to 5 for lesions on multiparametric prostate MRI, estimating the likelihood of clinically significant prostate cancer. Scores of 4 or 5 usually lead to targeted biopsy, while scores of 1 or 2 may allow suitable men to avoid biopsy.
Example: PI-RADS 4 lesion in the left peripheral zone; MRI-fusion biopsy scheduled.
Usage notes
Version 2.1 was released in 2019 by the ACR and the European Society of Urogenital Radiology. Diffusion-weighted imaging is the dominant sequence for the peripheral zone and T2-weighted imaging for the transition zone. MRI before biopsy became standard in many countries after trials such as PRECISION (2018).
