LVO stands for Large vessel occlusion, a neurology abbreviation.
Blockage of a major brain artery, such as the internal carotid, the first segment of the middle cerebral artery or the basilar artery, causing severe stroke. LVO strokes respond poorly to clot-dissolving drugs alone and are treated with mechanical thrombectomy.
Example: Prehospital LVO screen positive (RACE 7); bypass the nearest hospital for a thrombectomy-capable center.
Usage notes
EMS systems use LVO scales such as RACE, LAMS and VAN to decide whether to take patients directly to a comprehensive stroke center. The DAWN and DEFUSE 3 trials (2018) extended thrombectomy to as late as 24 hours in selected patients with salvageable brain tissue. CT angiography is the usual confirming test.
