PMH stands for Past medical history, a medical documentation abbreviation.
Section of the history listing chronic illnesses, past serious illnesses, hospitalizations and significant diagnoses. It gives context for the current problem and medication choices.
Example: PMH: type 2 DM, HTN, CKD stage 3a, prior MI 2018.
Usage notes
Also written PMHx. A good PMH includes dates and severity, for example "COPD on 2 L home oxygen", not just the label. It is usually followed by past surgical history, medications, allergies, family history and social history in a standard H&P.
